  Chapter 1

  Birth was much cleaner on TV than it was in real life. Or so Michael Harper, third year medical student, observed partway through his January rotation in Obstetrics.

  On TV, the supermodel, who’s pregnant with no extra fluid ballooning her ankles to the size of tree trunks, makes a groaning sound at someone else’s wedding. Her water breaks in a flash of clear fluid on her worst enemy’s shoes, and ten minutes later there’s a baby.

  Sure, the baby was usually fake or played by a six-month-old, and it was never dirty or rushed off to get resuscitated unless the plot called for it.

  Actual birth took hours, IVs, epidurals, screaming, and a large amount of body fluids, only some of which were blood.

  “The baby is crowning,” Dr. Carlisle said, hovering at Michael’s shoulder. “Harper, you ready to catch?”

  “Yes, ma’am,” Michael answered in his blue sterile gown. He had a mask, eye shield, gloves, and blue shoe covers over his green scrubs. He sat on the delivery stool waiting for the newest arrival of Amanda Wiggins’s brood, this being her fourth child. “Start pushing.”

  “Amanda, push on the countdown,” the ever-present delivery nurse encouraged, counting slowly to ten. The head slightly rotated and finally emerged. Michael reached out with his gloved hands to catch it.

  “Alright, the head is out,” Carlisle, the fourth year OB resident, encouraged. “Here comes the right shoulder. Deliver that, good. Good. Good.”

  The baby neatly landed in Michael’s hands with a splash of fluid and blood.

  This tiny scrap of humanity was still grey and hadn’t taken its first breath yet. They hadn’t cut the cord because the new peds and OB guidelines were giving the cord at least sixty seconds before clamping if the baby wasn’t in distress.

  Carlisle nudged Michael’s blue shoulder and whispered, “Say something.”

  “It’s a beautiful baby boy,” Michael announced. “He has ten fingers and ten toes.”

  The L and D nurses decided he must have done enough, and a minute had passed. They whisked the baby out of his hands and set it on the mom’s bare chest. Carlisle handed Michael a pair of umbilical cord clamps and scissors to cut between them. The resident kept up a stream of lighthearted conversation the whole time. “Congratulations, boy number four. He’s beautiful. Do you have a name picked out for him?”

  “Ryan. To go with Robert, Randall, and Reggie.”

  “That’s great,” Carlisle chimed in. No matter what the parents selected, the OBs insisted on telling everyone their name was great. Michael had questioned when someone named their child ABCDE, but once again, ‘it was great.’

  Michael was moved to the side to observe the mom while Dr. Carlisle delivered the placenta and afterbirth. Another part TV skipped.

  The nurses went about their business, slowly crowding Michael toward the door. As it was his first mostly solo delivery on his first official day in Labor and Delivery, Michael gave the mom a genuine smile. “Congratulations. Thank you for letting me participate.”

  “No problem. I swear with four, everyone in the hospital knows my name on this floor. That’s where Dad is today, caring for the other three.” She glanced at his disposable gown. “You may want to change that. Sorry.”

  Okay, so the nurses had even better reasons to hustle him out of the room. His gown was splattered, as were his scrub pants, socks, and shoes, despite the covers.

  Carlisle’s eyes were laughing behind her goggles. “Get cleaned up, Harper. Maternal Fetal Medicine Multidisciplinary rounds are going to start soon.”

  Before leaving the delivery room, Michael stripped off his entire ensemble, ending with the head cover. His short wavy hair got stuck in the weird fabric, though he resisted the urge to rearrange it.

  No way was he touching his head until he was in new scrubs and washed his hands up to his elbows.

  His shoes looked okay, but there was one drop of red blood visible on his white socks by the ankle. Would he have to walk around with the blood on his sock for his first call of thirty hours?

  Maybe he should ask the nurses at the front desk and they’d give him a pair of those no-slip patient socks. Two deliveries into the morning before rounds, and he was already on his second set of scrubs. Judging by the number of women in labor today, he might easily go through several more sets.

  He was on his way to the locker room when one of his medical student teammates, Raj Patel, popped out of an alcove.

  “Michael, I have to talk to you,” Raj said, urgency in his voice.

  Their team, composed of Raj, Michael, and Nora Borenstein, had been split between outpatient prenatal care, STI clinic, pelvic surgery, and gynecology. Today they reunited to begin their block on labor and delivery, which ended by March.

  “Everything okay?” Michael asked.

  “I took your advice,” Raj announced. “I got laid.”

  “Umm, good for you,” Michael said. Raj had been full of anxiety and ‘inappropriate’ sex thoughts for the past year and had asked for a medical work-up. It turned out he was having a growth spurt in his twenties and had associated horniness with it. Michael had told him to start dating and act more confident.

  “No, I REALLY got laid! I spent an entire weekend tied to her bed.”

  “Wow. One of the psych nurses?” Michael asked. Back around Christmas, during their psych rotation, Raj had gotten a jiu-jitsu injury, resulting in a badly bruised face. The psych nurses were all about it and had given him several phone numbers last rotation. Michael had recommended waiting a few months before calling them, lest he get into trouble, as the medical students had another six months of hospital rotations in third-year.

  “Nope, I had a New Year’s Eve hook-up at the Rocky Horror Picture Show.”

  “Oh, I hope you were safe.”

  “And safe worded. She was hard core. Those cuffs she used on me were real. She’s a cop.” Raj was almost dancing.

  “Oh, it wasn’t a metaphor.” Michael thought back to the first couple weeks between himself and his ex, Angela. They’d spent a lot of time in bed, unable to get enough, though handcuffs had not been necessary.

  They had other problems, however…

  “She only handcuffed me part of the time. Then she called over one of her friends, and things really got interesting.” Raj paused. “Is this normal?”

  Michael choked, “Which part?”

  “She got out a dildo, and we… are ladies into that?”

  Michael raised an eyebrow. “It sounds like she was into it. Were you?”

  “It was different. I learned tons, and it’s all thanks to you. You are my Obi Wan Kenobi of women and sex. You’ve opened up a whole new world for me. Or is it more Aladdin than Obi Wan?”

  “I am so proud,” Michael deadpanned.

  “Excuse me, can one of you pass on a message for me if you’re done talking?” an irritated voice asked.

  The two medical students turned to see Dr. Angela Perkins in the hallway, patiently waiting for them to finish their discussion.

  Her eyebrows were raised, and Raj had the grace to be embarrassed. “Sorry about that, Dr. Perkins. It won’t happen again.”

  “Excellent. If you’re continuing your conquests, please don’t date any of the nurses on OB until you’re off this rotation. It’s best not to date anyone in the hospital you have to see regularly, or it can mess up your career.”

  Also on TV, when a man runs into his secret ex-girlfriend, he’s at his very best. Not having a thoroughly inappropriate conversation while covered in blood and amniotic fluid with his hair standing sideways.

  “I promise I will date no one in internal medicine. Ever,” Raj stumbled to reassure her. He wanted to go into internal medicine for residency, so swimming in their dating pool could be career suicide.

  “Thank you for the unnecessary reassurance,” Angela said. “Can you tell the OBs? I have to run to the tenth floor to check an echo my tech’s doing. I should be back in twenty minutes for rounds.”

  She didn’t wait to hear Raj’s answer. “Yes, ma’am.”

  Once she was gone, Raj whispered, “Do you think she’s hot?”

  “What?” Michael almost swallowed his tongue.

  “Dr. Perkins. I figured since blondes are your type, you’d have an opinion.” Raj’s parents were both internal medicine doctors, and part of his medical student success strategy was being a gossip-absorbing sponge.

  “I don’t really wonder if she’s hot,” Michael told the truth. He didn’t need to wonder if Dr. Angela Perkins was hot because he’d spent plenty of time up close and personal with the short, curvy blonde. Nor did he need to wonder how delicious her skin tasted or if she made a soft mewing sound right before she orgasmed.
